Client Confidentiality
It is the policy of TASC of Northwest Ohio to maintain the confidentiality of all client information.
Federal law Code of Federal Regulations 42 part 2 protects all client records maintained by TASC of Northwest Ohio.
The Agency agrees not ot convey to a person outside the program that a client attends or receives services fro the program or disclose any information identifying a client as an alcohol or drug abuser unless:

  1. Client consents in writing.
  2. Disclosure is allowed by court order.
  3. Disclosure is made to medical personnel in a medical emergency.
  4. Disclosure is made to qualified personnel for research, audit, or program evaluation.
  5. Crimes commited by a client, either at the program or against any person who works for the program, or about any threat to commit such a crime is not protected.
  6. The is cause for suspicion of child abuse or neglect. Under Ohio law staff must contact the appropriate state or local authorities when suspicion of child abuse or neglect is suspected.

Violation of the Federal Law and regulation is a crime. Suspected violations my be reported to the appropriate authorities.
